Surfaces when a later audit, appeal, or legal challenge cannot verify what was decided and why. Requires that a record exists and that the gap is specifically reconstruction metadata on that record; excludes cases with no record (210), output-provenance omission (322), fabrication (512), merely hard-to-explain outcomes, and never-instrumented systems.
