A decision or action is recorded, but the record omits the specific metadata needed to independently reconstruct or validate that exact past decision (input hash, model/config version active at decision time, or per-decision reasoning). Surfaces when a later audit, appeal, or legal challenge cannot verify what was decided and why. Requires that a record exists and that the gap is specifically reconstruction metadata on that record; excludes cases with no record (210), output-provenance omission (322), fabrication (512), merely hard-to-explain outcomes, and never-instrumented systems.Distinct from
